Coordinating Care For a Healthier Community

Impact Health Tennessee is creating a network of Care Coordination Agencies and Community Health Workers focused on removing barriers to optimal health. This program collaborates with agencies that engage Community Health Workers to pinpoint vulnerable individuals and facilitate their access to medical, behavioral, and social services aimed at eliminating obstacles to health and wellness.
